Do you accept insurance?
- No, this is currently a self-pay practice. I have opted out of participation with Medicare and Medicaid, nor do I bill them for any services rendered. Likewise, I have chosen to forgo working with any other commercial insurance plans. You may use your FSA or Health Savings Account to pay for visits. You may also submit a superbill to your insurance in an effort to receive reimbursement. However, we cannot guarantee they will reimburse you for any services received from The Imperishable Seed, PLLC.
Will labs be covered by insurance?
- Unfortunately, labs are typically not covered by insurance as they are specialty labs that most mainstream lab companies do not offer. The Imperishable Seed, PLLC does have the ability to order labs from Quest, Labcorp, and CPL, but we cannot guarantee these labs will be covered since the Provider is not working with Medicare, Medicaid or Commercial Insurances. These lab orders may be submitted to your insurance in an effort to receive reimbursement, but we cannot guarantee reimbursement for any lab service rendered at The Imperishable Seed, PLLC.
Can the Functional Medicine Plans be paid for monthly?
- Yes, if you decide on a program that is right for you, you can pay monthly. You will sign a contract committing to the number of visit's you plan to complete.
Are there cancellation fees?
- Yes, if your scheduled appointment is not cancelled within 48 hours of the appointment or the client does not show up for their appointment, there will be a $150 cancellation fee.
- If a client chooses to cancel the remainder of their unused functional medicine appointments that were pre-determined and written in their contract, there will be a $150 cancellation fee.
- If you are a part of a monthly membership plan, you will be required to give 30 days' notice of cancellation.
Can I give my appointments to a family member if I do not plan to use them all?
- Yes! You may transfer the remainder of your appointments to an immediate family member. However, they will still need to complete an initial 90-minute visit at the established price and sign a contract committing to the number of appointments that remain.
Will you be my Primary Care Provider?
- No, you will need to maintain your relationship with your current PCP and Specialty services.
Can you order imaging and send referrals if needed?
- Yes! We can order imaging and refer out when needed and if it is in the best interest of the client.
How quickly can your clients expect to hear back from you when they are a part of your services?
- We will work to respond as quickly as possible, but no longer than 2 business days. Clients will be notified of Provider being unavailable due to illness, holidays, or vacation in the instance that these situations occur.
